AUTHORIZATION TO RELEASE INFORMATION:

APPLICANT REPRESENTS THAT THE REQUESTED LOAN IS INTENDED FOR BUSINESS OR
PROFESSIONAL USE, AND AGREES THAT UNDER NO CIRCUMSTANCES IS THIS TO BE
CONSIDERED AN APPLICATION FOR CONSUMER FINANCING.

By clicking submit, you authorize review of your personal credit profile from a national credit bureau. Such authorization shall extend to obtaining a credit profile in considering the application of the credit applicant and subsequently for the purpose of updating, renewal or extension of such credit and for reviewing or collecting the resulting account, and Lender and its assigns are further authorized to communicate with and exchange or disclose any of the Lender credit, financial or personal information that the Lender deems necessary to complete, service or enforce the Loan and any supporting ancillary agreements, including any additions, updates, modifications or other related transactions including
the Assignment, Sale or Securitization of a Loan. In addition to authorizing review of your credit profile from any national credit bureau, you also authorize your financial institutions and creditors to release any credit information required by Lender or its designee (and any assignee or potential assignee thereof). You further state that all application statements are true and complete.

IMPORTANT INFORMATION ABOUT PROCEEDURES FOR OPENING A NEW ACCOUNT: To help the government fight the funding of terrorism and money laundering activities, Federal Law requires all financial institutions to obtain, verify and record information that identifies each person who opens an
account. What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents. This is required in conjunction with our obligation under the USA PATRIOT ACT.
